Rajesh Kumar: Specialist Services Now to Be Provided at District Level<\/strong><\/p>\n<p data-start=\"232\" data-end=\"961\"><strong data-start=\"232\" data-end=\"245\">Dehradun :<\/strong> On the instructions of Chief Minister Pushkar Singh Dhami, the Uttarakhand government has started enforcing strict measures to curb unnecessary referrals of patients from government hospitals. Health Secretary Dr. R. Rajesh Kumar clarified that <strong data-start=\"491\" data-end=\"662\">no patient will now be referred from district and sub-district hospitals to higher institutions like medical colleges or larger hospitals without valid medical reasons<\/strong>. He stated that, following the Chief Minister\u2019s directives, it is being ensured that every patient receives <strong data-start=\"771\" data-end=\"849\">primary treatment and specialist consultation at the district level itself<\/strong>. Unnecessary referrals not only strain resources but also deprive patients of timely and appropriate treatment.<\/p>\n<p data-start=\"963\" data-end=\"1235\">Dr. R. Rajesh Kumar further informed that the Health Department has issued a comprehensive <strong data-start=\"1054\" data-end=\"1092\">Standard Operating Procedure (SOP)<\/strong> aimed at ensuring transparency, accountability, and medical justification in the referral system. The SOP emphasizes the following key points:<\/p>\n<ul data-start=\"1237\" data-end=\"2207\">\n<li data-start=\"1237\" data-end=\"1412\">\n<p data-start=\"1239\" data-end=\"1412\"><strong data-start=\"1239\" data-end=\"1294\">Referral Only in Case of Specialist Unavailability:<\/strong> A patient will be referred to a higher institution only if the necessary specialist is not available at the hospital.<\/p>\n<\/li>\n<li data-start=\"1414\" data-end=\"1621\">\n<p data-start=\"1416\" data-end=\"1621\"><strong data-start=\"1416\" data-end=\"1454\">Decision by Senior On-Duty Doctor:<\/strong> The on-duty doctor will personally examine the patient and decide on the referral. Referrals based on information received via phone or email will no longer be valid.<\/p>\n<\/li>\n<li data-start=\"1623\" data-end=\"1821\">\n<p data-start=\"1625\" data-end=\"1821\"><strong data-start=\"1625\" data-end=\"1674\">Immediate Decisions Permitted in Emergencies:<\/strong> In critical cases, the on-duty specialist can make life-saving decisions via WhatsApp or phone calls, but such decisions must later be documented.<\/p>\n<\/li>\n<li data-start=\"1823\" data-end=\"1995\">\n<p data-start=\"1825\" data-end=\"1995\"><strong data-start=\"1825\" data-end=\"1861\">Written Justification Mandatory:<\/strong> The referral form must clearly mention the reason for the referral, such as the unavailability of specialists or necessary resources.<\/p>\n<\/li>\n<li data-start=\"1997\" data-end=\"2207\">\n<p data-start=\"1999\" data-end=\"2207\"><strong data-start=\"1999\" data-end=\"2043\">Senior Officials to Be Held Accountable:<\/strong> In cases of unjustified or unnecessary referrals, the concerned <strong data-start=\"2108\" data-end=\"2139\">Chief Medical Officer (CMO)<\/strong> or <strong data-start=\"2143\" data-end=\"2181\">Chief Medical Superintendent (CMS)<\/strong> will be held accountable.<\/p>\n<\/li>\n<\/ul>\n<h3 data-start=\"2214\" data-end=\"2257\">Special Focus on Ambulance Management<\/h3>\n<p data-start=\"2258\" data-end=\"2681\">Dr. R. Rajesh Kumar mentioned that, as per CM Dhami\u2019s instructions, <strong data-start=\"2326\" data-end=\"2447\">clear guidelines have been issued regarding the use of ambulance services to ensure transparency in patient transfers<\/strong>. The <strong data-start=\"2453\" data-end=\"2478\">108 ambulance service<\/strong> will be used strictly under <strong data-start=\"2507\" data-end=\"2540\">Inter Facility Transfer (IFT)<\/strong>. Departmental ambulances will be deployed in a planned manner, and their technical condition will be reviewed to ensure operational fitness.<\/p>\n<h3 data-start=\"2688\" data-end=\"2746\">District-Wise Status of Ambulances and Mortuary Vans<\/h3>\n<p data-start=\"2747\" data-end=\"3283\">Currently, the state has a total of <strong data-start=\"2783\" data-end=\"2807\">272 &#8220;108 ambulances&#8221;<\/strong>, <strong data-start=\"2809\" data-end=\"2840\">244 departmental ambulances<\/strong>, and <strong data-start=\"2846\" data-end=\"2871\">only 10 mortuary vans<\/strong>. Certain districts\u2014such as <strong data-start=\"2899\" data-end=\"2952\">Almora, Bageshwar, Champawat, Pauri, and Nainital<\/strong>\u2014do not have mortuary vans. CMOs of these districts have been instructed to ensure immediate alternative arrangements. Dr. Kumar added that <strong data-start=\"3092\" data-end=\"3110\">older vehicles<\/strong>, whose registration age has completed <strong data-start=\"3149\" data-end=\"3167\">10 or 12 years<\/strong>, can be deployed as mortuary vans as per regulations.
